Biology of the Myocardial Vasculature.

Our present research program aims to identify the biological basis of the increased cardiovascular pathophysiology associated with advanced age in order to develop novel approaches to treat, and potentially prevent, cardiovascular disease in older persons.

Treatment of Patients in Cardiogenic Shock.

Peripheral Vascular Disease.

It has recently been shown in the SHS that low and high values of the ankle brachial index (ABI) independently predict cardiovascular (CV) events and death, including cardiac events without direct connection to peripheral arteries, in individuals without clinically overt disease. We have now proven the hypothesis that low and high ABI would be associated with echocardiographic markers of preclinical CV disease in 3,084 SHS participants who were free of clinical CV disease at the time. These abnormalities may contribute to mediating the known associations of both low and high ABI with cardiac events that are not directly related to the peripheral arteries that contribute to generating the ABI. We also extend this analysis to the cerebral vasculature.

Risk Factors for Cerebral Ischemia.

Research activities include the Thrombophilia In Cryptogenic stroKe (THICK) Study, with funding provided by a Patient-Oriented Career Development Award (K-23); analysis of population-based cohorts for which Cornell has participated as the Core Echocardiography Laboratory; and analyses of clinical data from the Echocardiography Laboratory on risk factors for cryptogenic stroke and embolic stroke in cancer. Training activities consist of coursework in genetic epidemiology at the Columbia School of Public Health and at the Southwest Foundation.

Epidemiologic Risk Factors for Cardiovascular Disease

Research activities include investigation of adipokines in the Cardiovascular Health Study, with funding from an AHA Clinically Applied Research Grant; evaluation of lipoprotein-associated phospholipase A2, with funding from Diadexus, in the Strong Heart Study (application pending); and the relative predictive value of albuminuria and glomerular filtration rate in the Strong Heart Study and the LIFE Study.

Premature vascular aging in chronic inflammatory diseases.

This area of research examines the impact of chronic inflammation, exemplified by SLE and rheumatoid arthritis, on vascular aging as manifest by preclinical atherosclerosis detected by carotid ultrasonography and vascular stiffening detected by applanation tonometry. These publications support the hypothesis that chronic inflammatory diseases are associated with premature vascular aging; analysis of longitudinal data is expected to provide strategies for interventional studies.
